Some of the items are not coming up under Debrief forms when doing Debrief

Expect to see the items in Debrief

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Log in Depot Repair responsibility
2. Create Repair Order
3. Create Repair Task, Debrief the task
