FRM-47023 Error When Click On Log And Notes Button (Doc ID 2852086.1)

Symptoms

On : 12.2.11 version,

When attempting to click on 'Log and Notes' button,
the following error occurs.

ERROR

  "FRM-47023: No such parameter named SOURCE_OBJECT_CODE exists in form CSCCCCRC"
  "FRM-47023: No such parameter named SOURCE_OBJECT_CODE exists in form CSXSRISR"


STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Navigate to (R) Service > Customer Management > Contact Center.
2. Search for the customer and click on Service Requests tab.
3. Click on 'Log and Notes' button. The mentioned error occurred
4. Navigate to (R) Service > Service Requests > Create Service Requests.
5. Query the SR and click on Workbench tab.
6. Click on 'Log and Notes' button. The mentioned error occurred
