Creating Volume Offer - Error: oracle.apps.fnd.framework.OAException: java.sql.SQLException: Initialization Failed (Doc ID 2875091.1)

Symptoms


Receive error when creating Volume Offer in Trade Management.  This is seen when system profile 'OZF: Default Offer Activity' is blank at all levels


Error
Set Profile option FND: Diagnostics to 'Y' to view Stack Backtrace

Error: oracle.apps.fnd.framework.OAException: java.sql.SQLException:
Initialization failed


Steps
The issue can be reproduced at will with the following steps:
1. Navigation: Oracle Trade Management User 
2. Trade Management > Trade Planning > Offers > 
3. Create new Volume Offer,  click Create (B).




Changes

 Upgrade from 12.1.3 to 12.2.9.
