R12: AP: Unaccounted Transactions Report Getting into Error "FRM-92101 There Was A Failure In The Forms Server" (Doc ID 2878142.1)

Symptoms

After 12.2.11 upgrade, submitting the program "Unaccounted Transactions Report" crashes the form with below erro:

FRM-92101 There was a failure in the Forms Server during startup. this could happen due to invalid configuration

Unable to communicate with runtime process

oracle.forms.net.ConnectionException: Forms session aborted: unable to communicate with runtime process.

Steps to Reproduce the Issue:

1. Goto Payables responsibility --> Submit new concurrent request

2. Program name: Unaccounted Transactions Report

3. Provide mandatory parameters and submit. Form will crash with above error.
