Symptoms
When importing orders, the following error is seen in the Corrections Form, creating thousands of records each time:

ORA-01403 no data found in Package OE_ACCEPTANCE_UTIL Procedure Default_Contingency_Attributes
STEPS

The issue can be reproduced at will with the following steps:
1. Order Management Super User Responsibility
2. Run Order Import
3. Errors for the imported orders on the OE Interface corrections screen
