Requisition Template Not Saving After Adding Line Item

Symptoms
When attempting to add a new inventory line to the current requisition template, the requisition template does not get saved.
It is expected that the requisition template would be saved with the new value.
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Navigate to purchasing module, --> setup --> purchasing --> requisition template
2. Query existing template
3. Add vendor name
4. Add a new line and save
5. New line is not saved.
6. Re-enter vendor name, and add line again, after a couple of tries it finally saves the new line.
