In EBS 12.2.4 Environment after applying patches and ADOP cycle submit request form hangs

After applying patches, cannot run any concurrent requests
submit request form hangs
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
Apply any patches and complete ADOP cycle
submit request from concurrent SRS window
Changes
Apply Patches and Run ADOP cycle
