Unable to create Iprocurement Requisitions due to Error trying to checkout

The issue can be reproduced at will with the following steps:
 
Steps to reproduce:
1. After applying several patches , the patch logs from the patches applied do not show any errors during application
2. Start a non-catalog request in iProcurement Requisitions
3. Enter the  required fields
4. Select Add item to Cart
5. Get the prompts for the Information Templates
6. Select Next, and then get the error
7. There is no way to return to the fields to add another item to the shopping cart.
