Symptoms
After applying patch 33643549:R12.CSM.C and 34735044:R12.CSM.C, the service request date is printed incorrectly in Service Report in Mobile Field Service.
STEPS

The issue can be reproduced at will with the following steps:
1. Log into Mobile Field Service.
2. Select a task.
3. Enter a debrief labor line.
4. Open the Service Report.
