URL Validation Failed When Opening Forms After EBS Upgrade

Symptoms
On : EBS 12.2.10 version, Forms for App Technology
While trying to access Forms, the following error occurs:
ERROR
-----------------------
URL validation failed. The error could have been caused through the use of the browser's navigation buttons.
If we try to open the form again, it opens without error.
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Upgrade the instance to 12.2.10
2. Login to application.
3. Open any form
BUSINESS IMPACT
-----------------------
The issue has the following business impact:
Due to this issue, users cannot open forms.
Changes
Upgraded the EBS application from 12.1.3 to 12.2.10.
