Attachment Icon Is Not Available (Greyed Out) In EBS Java Forms (Doc ID 2937303.1)

Symptoms

An end user wants to add an attachment in one of the forms that has been set up for a custom responsibility but the icon attachment, paper clip, icon is greyed out.

Furthermore, if they navigate to "View" on the toolbar, the "Attachments" option is also greyed out.
