Unable To Define New Problem Codes In The HTML Setup UI (Doc ID 2950418.1)

On : 12.2.10 version, Customer Support

When attempting to enter new problem codes in Customer Support Responsibility from the Html Setup Problem Code page, the following errors are thrown

Form Validation Failures,
A value must be entered for lookup code
A value must be entered for meaning

The issue can be reproduced at will with the following steps:

1) Customer support (R)
2) Setup(N) Definitions - Html Setup
3) Access the Problem codes page
4) Enter values for Code, Meaning and Description field and then click apply when the errors are thrown
