Getting error "FRM-92101: There was a failure in the Forms Server during startup" when Submit "Approve or Cancel Approval of Schedules" program .

The error is displayed on the Submit Request form as soon as the program is submitted.

FRM-92101: There was failure in the Form server during startup. This could happen due to invalid configuration. Please look into the web-server log file for details"

STEPS
-----------------------
The issue can be reproduced at will with the following steps:

1. Property Manage responsibility > View > Run request

2. Select "Approve or Cancel Approval of Schedules" program > submit the request

3. Error is displayed on Submit Request window.
