R12: AP: Unable to Create Vendor Sites "java.sql.SQLRecoverableException: No more data to read from socket" is Displayed (Doc ID 2958632.1)

Symptoms

When attempting to create a supplier site, the following error occurs:

Address and Site Creation - Unable to create address and sites because of the following error when trying to create the site in operating unit [United States OU]: [java.sql.SQLRecoverableException: No more data to read from socket].
Either resolve this issue and try again or deselect the operating unit with the error and continue.
Note: any new address requires at least one site in order to be created. 

The issue can be reproduced at will with the following steps:

1. From Payables Responsibility, go to Suppliers > Entry
2. Query the supplier
3. From Quick Update select Create Supplier Site
4. Select Address, Site Name and apply
5. Error is displayed, and site is not created
