iStore Address Book message "The city, county, state, and zip code combination is incorrect"

Symptoms
Trying to create a new address in the iStore Address Book, the following error is received:
"The city, county, state, and zip code combination is incorrect. Please correct them."
The location flexfield structure was set to 'State.County' in the Receivables system options for Tax.
The address validation was enabled by setting it to Error in the Receivables system options for Tax
