Symptoms
When attempting to create an employee, the following error occurs.
FRM-40735: ON-ERROR trigger raised unhandled exception ORA-01001
Note: After the error when trying to close the form getting the below error:
APP-FND-01564: ORACLE error 3114 in audfrm
Cause: audfrm failed due to ORA-03114: not connected to oralce.
the SQL statement being executed at the time of the error was: &SQLSTMT and was executed from the file &ERRFILE.

The issue can be reproduced at will with the following steps:
1. Using HRMS Manager responsibility.
2. Navigate to People > Enter and Maintain > Create New employee.
3. Click on save.
