Duplicate records are found in the System Extract output

The issue can be reproduced at will with the following steps:

1. Navigate to US Super HRMS Manager responsibility
2. Navigate to Processes and Reports > Submit Processes and Reports
3. Select Single Request
4. Select Extract Process
5. Enter the required parameters and submit the request
6. View the output
7. Notice there are duplicate records in the Extract output
