ORA-06508 Error When Attempting To Create Service Request (Doc ID 2977687.1)

Symptoms

After upgrading the test instance to R12.2.12 from R12.2.6, users cannot create Service Requests via the SR form. The following error message is displayed:
 

Errors
ORA-06508: PL/SQL: could not find program unit being called in Package CS_ServiceRequest_PVT Procedure Create_ServiceRequest.



Steps
(R) Service
(N) Service Requests > Find/Create Service Requests
Create SR
