Cannot Approve Requisitions - Form Errors With "APP-PO-14142: In exception of Start_WF_Process ()- PO_REQAPPROVAL_INIT1" and "ORA-20002: 3108: Root Process Could not be Determined for Item AZNM005" (Doc ID 300745.1)

Enter Requisition screen - POXRQERQ.fmb


Symptoms

When an Internal/purchase requisition is submitted for approval in the Enter Requisition screen in the core application with the Submit and Forward checkboxes enabled, the approval errors fails with the following error displayed on a popup error message window:

APP-PO-14142: In exception of Start_WF_Process ()- PO_REQAPPROVAL_INIT1 . Start_WF_Process:  In Exception handler:
ORA-20002: 3108: Root process could not be determined for item 'AZNM005/4527072-674547'. There may a problem with the item type selector function, or you may be required to supply a starting process name.
Cause: A SQL error has occurred in In Exception of Start_WF_Process(). The SQL error is &LSQL_ERR

Also an error occurs in the workflow with a notification to sysadmin :

Sequence of events
--------------------------------
1. Submit an internal/purchase requisition for approval from the Enter Requisition screen
2. Just after submitting the internal/purchase requisition for approval while still in the screen the error modal window appears.
