Error when attempting to create a salesperson: FRM-92101: There was a failure in the Forms Server during startup (Doc ID 3012207.1)

When attempting to create a new salesperson from an employee, the following error occurs.


FRM-92101: There was a failure in the Forms Server during startup. This could happen due to invalid configuration.
Please look into the web-server log file for details.

Java Exception:
oracle.forms.netConnectionException: Forms session <8,626> aborted: unable to communicate with runtime process.

The issue can be reproduced at will with the following steps:
1. Login with responsibility Order Management Super User
2. Navigate to: Setup > Sales > Salespersons.
3. Create new salesperson from employee.
4. The error message appears and the application closes.
