Quoting Creates Duplicate Sites in AR

Symptoms
AR (Oracle Receivables) has two active, primary bill-to and ship-to sites for the customer account.
However, Quoting creates two new active sites (bill-to and ship-to) for the same account.
This causes problems in AR and OM as the newly created sites does not have all the required setup such as customer class profile and credit limits.
