Patch 36050661 Failed While Executing Wfreprole.ldt FNDLOAD (Doc ID 3020681.1)

Symptoms

While applying patch 36050661, it failed while executing "Wfreprole.ldt" FNDLOAD.

FAILED: file wfreprole.ldt on worker 1 for product fnd username APPS.


Below error noticed in the failed worker log:

Error calling FNDLOAD function.

While checking the log US_wfreprole_ldt.log, below was the error message :-

Dumping LDT file /u01/install/APPS/fs2/xxxxx/appl/fnd/12.0.0/patch/115/import/US/wfreprole.ldt into FND_SEED_STAGE_ENTITY
Dumped the batch (FND_SECURITY_GROUPS STANDARD , WF_ROLE WF_EXTERNAL_ROLE_NOEBS_ACCESS ) into FND_SEED_STAGE_ENTITY
Uploading from staging tables
Error loading seed data for FND_LOOKUP_ASSIGNMENTS: ROLE_NAME = UMX|FND_SYSTEM_INTEGRATION_ANALYST, LOOKUP_TYPE = UMX_CATEGORY_LOOKUP, LOOKUP_CODE = INFORMATION_TECHNOLOGY, ORA-20001: Could not cleanup seed data table: APPLSYS.FND_LOOKUP_ASSIGNMENTS, CODE: -20001, MESSAGE: "ORA-20001: [FND_LOOKUP_ASSIGNMENTS|CLEANUP_ROWSET] Task failed, status=8, -8102: ORA-08102: index key not found, obj# 546411, file 50, block 14977 (2)"
