When Adding Data to a Nonconformance Plan Error java.sql.SQLSyntaxErrorException: ORA-00904 is Thrown (Doc ID 3024443.1)

Symptoms

While attempting to add a record to a nonconformance plan, the following error message is seen:

Error: Can not execute the action on element <Collection Element> because of the following error: Application: FND, Message Name: FND_GENERIC_MESSAGE. Tokens: MESSAGE = java.sql.SQLSyntaxErrorException: ORA-00904: "<Based on Value Entered in Formula>": invalid identifier ; Please check the 'Assign a Value' action in the 'Collection Plans' form.

The issue can be reproduced at will with the following steps:
1. Quality > Workbench > Nonconformances
2. Find the plan and select it.
3. Select EDIT.
4. Select ADD DATA and page errors.
