11i: Why Are Some OIE Users Not Receiving Notification Emails ?

A few users are not receiving the notification emails from the OIE application even though the expense reports submitted by them are approved/rejected by their supervisors.

Note that Workflow and Email are set up properly and that other users do receive notification emails.   Why is this occurring?
