When attempting to apply the 12.2.13 upgrade patch 34776655,
the following errors  are shown in the adworker patch log file related to the compilation of form GMDQMASS.fmx

PL/SQL ERROR 201 at line 65, column 33
identifier 'ITEM_ID' must be declared
PL/SQL ERROR 0 at line 65, column 5
SQL Statement ignored

Compiling package specification GMDQMASS...
  No compilation errors.

Compiling package body GMDQMASS...
Compilation error on package body GMDQMASS:
PL/SQL ERROR 302 at line 279, column 42
component 'QC_LAB_ORGN_CODE' must be declared
PL/SQL ERROR 0 at line 274, column 5
SQL Statement ignored

1. Applying upgrade patch 34776655
2. Review  adworker patch log files for errors.
