Duplicate Requisition Line Numbers

Symptoms
When making an amendment to a requisition by adding an additional line, the application duplicates (CLIN) requisition numbers instead of adding the line to the requisition at the end of the list of requisition lines. For example, the CLIN line number 008 is duplicated and the CLIN line number 0012 is missing. The sequence after the amendment is 0001 - 0008, 0008-0009 - 0011, 0013 - 0015.
STEPS

1. Log into E-Business Suite.
2. Open iProcurement, open a particular requisition, and add a line.
