Contract Completion Statement Boxes 5,10A Have Incomplete/Unchecked Checkboxes (Doc ID 3041563.1)

Symptoms

On 12.2.10 version, Award/PO

When generating the contract completion statement using the FSMS "Manage Closeout" function and the KO electronically signs it in FSMS, in the resulting PDF (View Contract Completion Statement PDF) the following boxes on the resulting form (DD Form 1594) are incomplete/unchecked:

Box 10a:
  ==> The Box Labeled "Date Shown in Item 9d Above" should be checked if no comments/remarks were added in FSMS. In this closeout, no comments/remarks were added but the system did not check this box.
  ==> The Box Labeled "Date Shown in Item 10e Below" should be checked if comments/remarks were added in FSMS. Again, in this closeout, no comments/remarks were added.

FSMS functionality does not include any boxes/radio buttons for users to select to check the appropriate boxes.

Steps to reproduce:
 1. Go to CLM Purchasing responsibility > BWC > Awards
 2. Open award 70Z0G322FMSPL0004.
 3. Choose View Contract File from the Actions pull down list and click Go.
 4. Click on the blue link View Contract Completion Statement PDF.
 5. Notice that the check box for section 10a ALL PURCHASING OFFICE ACTIONS REQUIRED HAVE BEEN FULLY AND SATISFACTORILY ACCOMPLISHED.
     CONTRACT FILE OF THIS OFFICE IS HEREBY CLOSED AS OF: DATE SHOWN IN ITEM 9d.
     ABOVE is not checked even though the date in box 9d. DATE is filled in.
