Symptoms
When Lookup is created from EBS Forms, Application Object Library --> Lookups--> Common is having view_application_id as 3.
Where as Lookup created from OAF Page, Functional Administrator --> Core Services --> Lookups , is having different view_application_id as XXXX, not same as Form view_application_id
