Symptoms

Workflow notification mailer not sending out the Email notifications  or  workflow email notifications delayed in sending out due to the interruption of the error seen on the log.

Mailer logs show error [fnd.common.ErrorStack]:FND_SESSION_ICX_EXPIRED by itself or sometimes lead by or followed by one of the below:

Problem Obtaining the HTML Content oracle.apps.fnd.wf.common.HTTPClientException
oracle.apps.fnd.wf.mailer.SMTPMessageHandler.open()]:Unable to connect to transport
421 4.4.2 Message submission rate for this client has exceeded the configured limit
The sender has exceeded the maximum number of messages they are allowed to send via client submission either per minute or per day
oracle.apps.fnd.wf.mailer.NotificationFormatter.handleResEndTag]:Problem obtaining the HTML content oracle.apps.fnd.wf.common.HTTPClientException: Unable to invoke method HTTPClient.HTTPResponse.listHeaders caused by: java.net.ConnectException: Connection refused
 fnd.wf.bes.PLSQLQueueHandler]:dequeue() :Read timed out
Response Code {502} Reason Line{Bad Gateway}}
