Launching Forms From OA Framework Homepage Fails (Doc ID 305903.1)

Symptoms

User Symptoms
After logging into the OA Framework Homepage, the login is VERY slow.

Eventually the Self-Service Menu does render.

You can select a responsibility, but selecting a forms based function does nothing, browser just sits there as if hung. The java applet window for jinitiator never opens.
No error is presented.

Login using direct forms login (/dev60cgi/f60cgi) does work.

WebServer Symptoms
Do a count of the number of http processes
On the web server do:
ps -ef | grep iAS | wc -l

On a standard install we would expect about 30 processes returned.
If you quickly hit the httpd.conf limit of processes (by default this is about 500)

iAS Log files
After setting up iAS logging see Note 230688.1 <Note:230688.1> you collect the following information

[info] [client ] proxy receive timed out
[info] server seems busy, (you may need to increase StartServers, or Min/MaxSpareServers), spawning 8 children, there are 0 idle, and 462 total children

access_log
----------
Shows multiple repeated requests like:
"GET /pls/TEST/fnd_icx_launch.launch?resp_app=SYSADMIN&resp_key=SYSTEM_ADMINISTRATOR&secgrp_key=STANDARD&start_func=FND_FNDCPQCR_SYS&other_params= HTTP/1.0" 200 -

Changes
