Symptoms

When attempting to create a service request with a request type that triggers the auto creation of tasks, the following error occurs:

FRM-40735: ON-INSERT trigger raised unhandled exception ORA-01086

The issue can be reproduced at will with the following steps:
1. Log in with Service responsibility.
2. Go to Create Service Request form, enter a request type that triggers the auto creation of tasks and the rest of mandatory information then save.
