Symptoms
The following error occurs when attempting to process invoices/payments using the concurrent program Create Accounting:
Error Number 95265
Error Message: It is not possible to create an account mapping based upon the code combination 0 using the chart of accounts mapping <coa_sla_mapping_name> due to INVALID_FROM_CCID. Please update the chart of accounts mapping or use a different mapping.

The FND Debug log captures the error for secondary ledger_id as:
The issue can be reproduced at will with the following steps:
- Responsibility Payables Manager
Navigation: Menu > View > Requests > Submit a New Request... > Single Request - Select from List of Values (LOV) > Create Accounting
Provide the End Date parameter > [B] Submit > Click on Output - Notice that invoices/payments are not accounted and reported with the above error message.
