In Contract Lifecycle Management, CLM Award Mods Disappeared From the System After Workflow Error

Symptoms
Whenever a there is more than one modification at the same time for any specific document (Award/IDV), and one of those Modifications get signed and approved, it causes the other modification to disappear.
The issue can be reproduced at will with the following steps:
1. Create a Base IDV. Approve it.
2. Create Modification #1. Submit it.
3. While Modification #1 is in Pre-Approved status, create Modification #2.
4. Sign Modification #1, which will change the status from Pre-Approved to Approved.
5. Draft Modification #2 disappears after mod #1 was approved.
6. If you create a new modification, the serial number skips even though you are no longer able to access this document.
