Find Pre-Implementation in ECO implementation program is failing and change order is not getting implemented, the following error occurs:

Steps
The issue can be reproduced at will with the following steps:
1. Create an item after using Development Manager > Create Item
2. Approve a change order that was assigned to the above item with Item rules combination assigned.
3. Change order stuck in Approved (Implementation failed) status.
