Symptoms
When attempting to upgrade application from 11i to 12.2 the upgrade driver fails in inviuic1.sql, with the following error:
ERROR at line 1:
ORA-20001: Error in update of default category id ORA-01422: exact fetch
returns more than requested number of rows
ORA-06512: at line 119

The issue can be reproduced with the following steps:
1. Apply the R12.AD.C (<Patch#10117518>) with AD CUP13 (<Patch#33526872>:R12.AD.C) for Upgrading to Release 12.2
2. The inviuic1.sql script fails with the above mentioned error.
