Symptoms
You are unable to select a customer to put on an opportunity. The following error message is received:
One or more transformations used by the match rule have not been staged.
Please confirm that all required transformations have been staged.
Navigation to reproduce this issue:

Sign on with Sales User responsibility
-
Scrolled down to Top Opportunities section
-
Hit 'Create'
-
Enter the following: Opportunity Name, Methodology
at Customer field > hit 'Select'
enter partial name in 'Name' field > hit 'Go' -
Receive error message.
