Primary Contact Field Blank In Employee Self Service

In Emergency Contacts region of the personal information page,  no contact is shown with Primary Contact as Yes, even though primary contact flag is checked for a contact relationship in forms.

On updating a contact to set primary flag in self service, following error is thrown.

ORA-20001: This employee already has a primary contact. Cause: You are  trying to enter more than one primary contact for an employee. Action:  Uncheck the Primary Contact check box and save your contact information. ORA-06512: at "APPS.

In some cases, it is also observed that relationship flag is blank in Emergency Contacts table of the personal information overview page.
