Cannot Query Any Plans in the Monthly Premium Form (Doc ID 330466.1)

Symptoms

Have set up premiums; however, the Monthly Premium form is not populating.
Navigation: People > Total Comp Contribution > Monthly Premiums

From the Monthly Participant Activity form, can see that there are premiums for some plans, but no plans can be queried on the Monthly Premium form.
