Workflow 'Rma For Credit W/ Approval' Notification Not Routed

Symptoms
RMA was entered on an order. The order type was created using a seeded workflow 'Line Flow - Return for Credit Only with Approval' for RMAs (returns). The profile 'OM: Notification Approver' is set to US OM Superuser. The order was booked.
The return line progresses to 'Awaiting Approver' but the notification is not getting routed to the appropriate responsibility. WF Notifications menu option returns: There are no notifications for XXX, where XXX is the user.
The issue was verified by reviewing the script output from concurrent program Diagnostics: OM Order Information. The return lines are stuck in status APPROVE_WAIT_FOR_H.
