"null Cannot Get Connection to the Database" Error Occurs During Login To Applications 11i (Doc ID 332472.1)

Symptoms

You are unable to login via AppsLocalLogin.jsp or ICXINDEX.htm. The following error occurs:

null Cannot get Connection to the Database

The error page displays null in the login fields, instead of Username, Password, etc..

A Framework error occurs with the details:

oracle.apps.fnd.framework.OAException: Application: FND, Message Name: FND_GENERIC_MESSAGE. Tokens: MESSAGE = java.sql.SQLException: Io exception: Broken pipe;  (Could not lookup message because there is no database connection)

JDBC Connection Pool test shows "0" available connections, i.e:
http://<hostname>.<domainname>:<port>/OA_HTML/jsp/fnd/AoljDbcPoolStatus.jsp
