Account Generator (FAGDA) Failed To Generate A New Code Combination

FAGDA
Symptoms
Depreciation (FADEPR) and Gain/Loss (FARET) fail due to an account combination that would violate a Cross Validation Rule (CVR).
The log file displays the following:
Account Type: COR_LOSS
Book: XYZ
Distribution ID: ####
Concatenated Segments: XXXX.XXXX.000.000.0000.0000.0000.0000.000000
Cause: The code combination for the generated segment does not exist and cannot be created because it would contain invalid segment values or because it has been disabled.
Action: Please review the following message for a detailed explanation of why the account could not be generated.
