Symptoms

Bill Presentment Super User -> Templates -> Query on a template -> Click GO -> Choose your custom template
Click on Interactive Preview
Enter Invoice Number From and To for the transaction in question  Click GO
Click on Online Print Preview
Document shows but has red 'x' in upper left hand corner

Changes

Bouncing the middle tier and apache server didn't make a change.  The red X still shows on the Print Preview
