Cycle Count Entry requires Approval even though setup does NOT require approval

Symptoms
Certain cycle count entries are required to be approved even though there are no setups that designated required approval.
Cycle count approvals are not set at Cycle Count, Item and Cycle Count Classes. The result is that certain cycle count entries are randomly moving to approval from cycle count entry and the Application is requiring these to be approved to complete cycle counting.
Profile options and settings:
All Subinventories are reservable
The adjustment quantiy is negative
For all organizations 'Allow Negative Balances' is checked
Inventory Organizations are not WMS enabled
Automatic recount option is NOT set for all cycle counts.
