How To Be Able To Default Appraisal Setup Details Values

Goal
How does to default appraisal's setup details values as follows:
- Period Start Date = 01-Jan-2006
- Period End Date = 01-Nov-2006
- Template = specific appraisal_template_id number
Before family pack K rollup it worked fine.
After family packs K rollup no default values appear.
Steps to Reproduce :
Navigation:
Manager self service ->
Appraisals ->
create standard appraisal ->
press Action icon for an employee ->
get to Setup Details page, where you should see the default values, but they do not appear.
