Vacation Rule Sends Approvals Twice Even if Approved Once

Symptoms
Using AME to decide the list of approvers using supervisor hierarchy for all self service (Employee, Manager, iRecruitment) modules.
One of the top approvers (Approver A) has created a vacation rule and delegated all the notifications to one of his direct report employee's (Approver B).
If the initiator makes any self service transaction (create vacancy), it goes through the hierarchy for approval with Approver B first and then Approver A as last approver.
When Approver B approves the transaction the first time, another approval request notification for the same notification is sent (as a result of vacation rule delegation from Approver A to Approver B).
As a HR business user, it does not make sense to approve the same transaction twice.
