Unable to access forms getting APP-FND-01926 error (Doc ID 391243.1)

Attempts to access forms results in the following error:

APP-FND-01926: The custom event WHEN-LOGON-CHANGED raised unhandled exception: ORA-06508: PL/SQL: could not find program unit being called.

The above could be the result of patching on the forms server side as well as not properly regenerating forms libraries.
