"Answer Question" Link Not Working In More Information Request Email

Symptoms
On 11.5.10.2 in Development:
"More Requested Information" email notification displays "Answer Question"
link instead of Applications Login page.
The issue can be reproduced at will with the following steps:
1. Create Request
2. As an approver click on "Request Information" button on the approval notification.
3. The receiver receives the email notification.
4. Click on "Answer Question" link in the email notification.
