Getting FRM-41800 when navigating to any LOV and Focus Lost

Symptoms
When trying to access any LOV in any form within Oracle Forms in Oracle Applications 11i the following error is occurred:
"FRM-41800 List of values not available for this field".
The only workaround is to log out and log in again, once this is done the forms can be used without issue until the issue occurs once again.
Just closing and reopening the form does not resolve the issue.
