Custom Pack Slip Document not created, or shows no data returned

Symptoms
Customer has a customized version of the Packing Slip Report. The report has been defined using a customized program name and a customized short name. When the "Print Document Set" button is pressed from the Shipping Transactions form, deliveries tab, the customized packing slip printed does not have a packing slip number and does not show any information.
The issue does not occur if:
1. The customized pack slip executable is called using the Oracle seeded program name.
2. The Ship Confirm button on the Shipping Transactions form is selected, rather than the 'Print Document Set' button.
