11i APPS User Account Is Locked While Applying a Patch. Patch Fails With "ORA-28000" or "ORA-01017: invalid username/password; logon denied"

When attempting to apply an applications patch after upgrading the database from 9i to 10g(10.2.0.2) the following error occurs:
ORA-28000 account locked on the apps account
Also in some cases during and upgrade to 12.2 as per <Note 1320300.1> users may encounter the following error in the adworker log:
ORA-01017: invalid username/password; logon denied
occurred while executing the SQL statement:
Upgraded the database from 9i to 10g(10.2.0.2)
